2011 Conference Registration Information

Conference Registration Fee:

$275 (member) and $290 (non-member) per person, includes all conference sessions, breaks, 2 lunches, 2 dinners, and special events.

Registration deadline is Wednesday, September 28, 2011.

Step OneRegistration

Fill out the registration form and either email or fax it to: phillipss@ecu.edu / 252-328-0968. If paying by check please include the form with payment.

Registration:

A $30.00 late fee will be charged for registrations received after Wednesday, September 28, 2011.

Additional Guest Meals and Entertainment:

Additional guest meals and special events must be included on the registration form by Wednesday, September 28, 2011.

 
Refund Policy:

Partial refunds will be made for cancellations until Wednesday, September 28, 2011. A $30.00 administrative fee
will be deducted and no refunds will be given for meals or venue admissions. Notification of cancellations must be in writing.

 
Disability Accommodations:

Individuals with disabilities should notify the Institute office by Wednesday, September 28, 2011, if special accommodations are needed for conference participation.

 
Conference Desk Check-In Hours:

Wenesday, October 12, 2011: 1:00 - 4:00pm at the Fifth Third Conference Room, 315 West Broadway, Louisville, KY.

 
Air Travel:

the Louisville International Airport (SDF) is just 10 minutes from downtown Louisville,  It is served by American, Continental, Delta, Froniter, Southwest, United, US Airways and Vision.

 
Airport Transportation:

The Brown Hotel offers free shuttle service to/from the airport, give them your airline information when you register. Car rental agencies available at the Louisville International Airport include: Avis, Budget, Dollar, Enterprise, Hertz, National/Alamo and Thrifty.

Company Displays

Please bring your company brochure materials for display in the lobby at 315 West Broadway during the conference check-in hours on Wednesday October 12 from 1:00 – 4:00 pm. The displays will remain in the lobby for the duration of the conference.

Conference Hotal Information

The conference hotel is the historic Brown Hotel at 335 West Broadway, Louisville,, KY 40202. The Brown Hotel special group rate for the IOD is $115 per/night plus tax. The deadline to register at the special IOD group rate is Saturday, September 10, 2011. To register, call 502-583-1234 or (800) 888-5252.
